Not grounding the shield properly
A shield that isn't grounded acts as an antenna, potentially making noise worse. In most cases, ground the shield at one end only to avoid ground loops (unless using high-frequency signals).
Over-specifying
Don't pay for shielding on simple DC power lines or digital signals in quiet environments. It adds unnecessary cost, weight, and stiffness.
